Health Screening and Prevention Services

  • HIV testing & counseling (rapid testing with federally compliant privacy protections)

  • HIV prevention services, including PrEP & PEP prescriptions, consistent with clinical guidelines

  • Sexually transmitted infection (STI) testing & treatment with monitoring for equitable access outcomes

  • Hepatitis C screening & treatment with outcome reporting aligned to federal program requirements

  • Harm reduction services: safe syringe access and disposal, education, and linkage to care with safeguards to ensure compliance with federal, state, and local regulations

Primary & Preventive Care

  • General health check-ups, with documentation aligned to federal quality and reporting standards

  • Chronic disease management (diabetes, hypertension) with monitoring of treatment

  • Immunizations and vaccinations are administered in compliance with federal and state guidelines 

  • uberculosis (TB) testing, with reporting consistent with public health requirements 

  • Women’s health services (Pap smears, contraceptive care, pregnancy testing), with safeguards to ensure equitable access and privacy protections 

Inclusive Health Services

  • Hormone therapy and transgender healthcare, delivered in accordance with established medical and federal regulations

  • Confidential mental health services and peer support groups

  • Sexual health education and outreach programs, emphasizing evidence-based practices

Substance Use and Behavioral Health

  • Substance use counseling and treatment referrals, documented in line with federal and state program requirements 

  • Mental health therapy and psychiatric services, with safeguards for confidentiality and evidence-based care

Integrated Community Health Approach

  • Key Services Include: 

  • Behavioral Health: Counseling and clinical support for mental health conditions and substance use, with pathways to treatment and recovery. 

  • Street Medicine: Mobile medical care delivered directly to individuals experiencing homelessness—on the streets, in encampments, and in shelters—ensuring outreach-based access to essential health services. 

  • Health Screenings: Routine screenings for HIV, STIs, diabetes, and other conditions, with a focus on early detection and linkage to care. 

  • Basic Dental Services: Dental screenings are currently offered in partnership with NJCRI’s affiliated physician group. 

  • Community Resource Navigation: Referrals and support for transportation, nutrition access, and other social services that impact health outcomes. 

  • This integrated framework is designed to reduce disparities and promote consistent access to preventive and primary care, with service delivery guided by public health data and community needs assessments.
