Supportive Services

Our integrated programs help individuals and families get access to healthcare, housing, and other essential resources. Through coordinated case management and on-site services, we support stability, recovery, and long-term well-being. 

Support & Care Coordination

  • Medical & Behavioral Case Management: Helps connect medical care, mental health services, and social support so clients receive consistent care and follow their treatment plans. 

  • Case Management & Housing Referrals: Connects individuals to emergency, short-term, and long-term housing options, while tracking referrals and outcomes. 

  • Financial Emergency Assistance: Provides short-term financial help for urgent needs such as rent, utilities, and other critical expenses, based on program guidelines. 

Access & Navigation

  • Insurance Navigation: Helps clients apply for, keep, and understand health insurance and benefits. 

  • Re-Entry Support: Supports individuals returning to the community by helping them access healthcare, housing, jobs, and social services. 

  • Recovery Court Collaboration: Works closely with Recovery Court participants to coordinate care and case management services. 

Advocacy & Resources

  • Legal Advocacy: Connects clients to trusted legal aid services for help with housing, benefits, or healthcare issues. 

  • Medical Transportation: Offers transportation assistance for medical appointments, pharmacy visits, and other essential services. 

  • On-Site Lab Services: Provides convenient lab testing on-site for faster and more accurate results. 

  • On-Site Pharmacy: Offers prescribed medications on-site to help clients stay on track with their treatment.
