Our integrated programs help individuals and families access healthcare, housing, and essential resources. Through coordinated case management and on-site services, we promote stability, recovery, and long-term well-being.

Support & Care Coordination

  • Medical & Behavioral Case Management: Coordinates medical, behavioral, and social services to promote continuity of care, compliance with treatment plans, and data-informed outcomes.

  • Case Management & Housing Referrals: Connects individuals and families to emergency, transitional, and permanent housing with verified referral tracking and outcome documentation.

  • Financial Emergency Assistance: Offers time-limited financial support for urgent needs, including rent, utilities, and critical expenses, in accordance with established eligibility and documentation standards.

Access & Navigation

  • Insurance Navigation: Assists clients in applying for, maintaining, and verifying health insurance and benefits through federally compliant processes.

  • Re-Entry Support: Assists returning community members in accessing healthcare, housing, employment, and social services, while monitoring equitable service delivery and mitigating risks.

  • Recovery Court Collaboration: Coordinates case management and treatment for Recovery Court participants through formal data-sharing and performance monitoring agreements.

Advocacy & Resources

  • Legal Advocacy: Refers clients to accredited legal aid resources for housing, benefits, or healthcare-related issues, ensuring informed consent and conflict-of-interest safeguards.

  • Food Pantry: Provides equitable, confidential access to nutritious food, tracked for inventory integrity and community need assessments.

  • Medical Transportation: Offers documented transportation support to medical appointments, pharmacies, and essential services to reduce barriers to care.

  • On-Site LabCorp Services: Delivers accessible lab testing on-site with verified reporting protocols to ensure timely, accurate results.

  • On-Site Pharmacy: Dispenses prescribed medications directly on-site to improve adherence, with controlled substance and audit compliance monitoring.
