The Chronic Disease Self Management Program
The Chronic Disease Self-Management Program (CDSMP) was developed at Stanford University’s Patient Education Resource Center as a collaborative research project between Stanford and the Northern California Kaiser Permanente Medical Care Program. The purpose of the program is to help people with chronic conditions and/or their caregivers to develop the necessary tools to effectively self-manage their or their loved ones’ chronic condition.

People with chronic conditions have similar concerns and problems.

People with chronic conditions must deal, not only with their disease(s), but also with the impact these have on their lives and emotions.

Lay people with chronic conditions, when given a detailed leaders manual, can teach the CDSMP as effectively, if not more effectively, than health professionals.

The process or way the CDSMP is taught is as important, if not more important, than the subject matter that is taught.
At NJCRI our CDSMP is
titled Live Healthy,
Live Happy. The
Live Healthy, Live Happy workshop series is
comprised of six, 2 ½ hour workshop sessions held once a
week for six weeks. To date, NJCRI has hosted two
Live Healthy, Live Happy workshop series. In
addition, we have also run a Peer Leader Training program to
train past workshop participants and other paraprofessionals
to facilitate the Live Healthy, Live Happy
workshops.
As a participant in the Live
Healthy, Live Happy workshops you’ll get the
support you need, find practical ways to deal with pain and
fatigue, discover better nutrition and exercise choices,
understand new treatment choices, and learn better ways to
talk with your doctor and family about your health. If you
have conditions such as diabetes, arthritis, high blood
pressure, HIV/AIDS, heart disease, chronic pain, anxiety,
etc., the Live Healthy, Live Happy workshop
can help you take charge of your life.
